I hereby acknowledge the risk and accept any and all responsibility for any injury or loss which is caused by my participation in Jiu-Jitsu training at Ground Work Jiu-Jitsu. I am at least 18 years of age. If I am under 18 years of age, a parent or guardian will sign below on my behalf.
Acknowledges and fully understands that each participant will be engaging in activities requiring physical contact, that by their very nature involve risk of serious injury, including permanent disability, paralysis, and death, and severe social and economic losses, including damage or loss of personal property, which might result not only from their own actions, inactions, or negligence but also the actions, inactions, or negligence of others, the rules of play, the conditions of the premises or of any equipment used. Furthermore, that there may be other risks not known to them or not reasonably foreseeable at this time.
Certifies that the participant, to their knowledge is in good physical condition and has no disease or injury that would prevent or impair participation in this activity, and that no instructor, coach, manager, doctor, nurse, medical professional, athlete, trainer, or other person has advised the participant not to compete in a contact sport for any reason, and not to participate in this activity.
I have had an opportunity to have this agreement independently reviewed before signing.
Therefore, I do hereby for myself, my heirs, executors, and administrators release and hold harmless Ground Work Jits LLC, d/b/a Ground Work Jiu-Jitsu and its board members, instructors, other students, guests and any guest instructors from any and all liability, negligence, claims, demands, actions or causes of actions, or caused of actions resulting from any participation in this sport/activity/class and associated activity.
The parties chose to apply the law of the state of CT to this waiver.
